APPLY BY SENDING US THE FOLLOWING FILES FOR OUR RECORDS:
1) A copy of your valid practitioner's license.
2) A copy of your practitioner's clinic or practice letterhead.
3) A copy of your practitioner's driver's license or personal identification card. (For this item you can block out the ID numbers for your privacy, if you wish!)
*You can use a simple cell phone camera to record the images and then email us the attached files to send us your documents. As long as the images are clear and not blurry, these files are sufficient for our purposes.
Once we have these items for our records, we can issue your account, once you have created an account on our web site.
We will make Private Labels for your Practice or Clinic to facilitate the promotion of these items, as you wish. When you make your first purchase, please send us a message letting us know which information you would like to see on your personalized labels. There are 2 short lines where we can include your Practice Name, Dr. Name, Phone Number and Web Site Address for your patients to contact you and reorder your new products.
There is a small minimum purchase quantity of ten (10) or twelve (12) of each item, in order to obtain your special discounts, once we have approved your account.
REGARDING WEB SITE ORDERS / DISCOUNTS FOR DOCTORS
Once we have approved your Doctor's Account, you can then log in to your account at any time and you will always have access to your discounted pricing. After choosing the item you wish to purchase, simply enter the Minimum Order Quantity (10 or 12) in your shopping cart. Your Special Discounted Pricing will then be automatically activated!
REGARDING WEB SITE ORDERS / DISCOUNTS FOR PATIENTS
When you place your order here on our web site your Credit Card will NOT be automatically charged! You will first receive an automated email receipt. Your first receipt will NOT reflect your "First time customer 10% discount". First time customer discounts will be applied by an actual person, after we retrieve your payment information from our secure servers. So please be patient to receive your final (or second) receipt. Once you have received your second receipt, your final discounted pricing will be available on your invoice and in your order history, in your account.
When you place your order on our web site, all items will be shipped as quickly as possible from our Fulfillment Center in South Florida.
